A crucial first step towards an impactful education and success later in life is showing up to school! Get Up & GO (GOGO) is a community-based program that addresses chronic truancy by partnering with students and their families to get their children to school every day. The program works with students and families to identify barriers to school attendance. We provide truancy support specialists who develop a plan alongside the family to address the family’s needs to ensure their children attend school daily. The program also has an incentive program geared specifically towards middle and high school students to foster a positive attitude towards school culture and school attendance.
All For One is an Intensive Case Management Program that works with youth, 7 to 17 years of age who are engaging in behaviors such as skipping school (truancy), leaving home without permission, not returning home by curfew, exhibiting extreme disobedience inside and/or outside the home or have been diagnosed with a type of conduct disorder. The goal is to provide early intervention services to the youth and their family to prevent contact with the juvenile justice system, child welfare system or becoming a victim of human trafficking. We provide highly trained Case Management Specialists to work intensely to identify the root cause of these behaviors. The Case Manager then wraps the child and family with community resources and natural supports to reduce or eradicate these behaviors.
Swaddle Me is an early childhood wraparound system that provides intervention services and support systems for preschool children ages 2-6. Our number one goal is to ensure that all children and families have access to comprehensive, high-quality early childhood services that promote child-centered well-being and school readiness.
Swaddle Me uses a family-centered approach by engaging with the family to understand their lives, goals, strengths, and needs to develop a Plan of Care. The goals developed in the Plan of Care are tailored to the priorities discussed with the family during the ongoing assessment process. Swaddle Me includes case management services for families who may be at risk of experiencing more significant problems, intensive in-home services geared towards preventing family disruption, healthy living practices, and connections to additional community-based resources.
Services are provided in the family’s natural environment such as a home, learning center, educational setting, playgroup, or at a local library.
The Sooner, The Better (TSTB) is a short-term functional family support service that provides a 6-month family counseling service that focuses on adolescents who have begun to exhibit risky and defiant behaviors. Functional Family Therapy addresses adolescents displaying a new pattern of disobedience, truancy, curfew violations, leaving home without permission, and legal involvement. The goal is to catch these behaviors at the onset, discover the reason for change in behavior, and work to eliminate the behaviors before they escalate. A therapist will be assigned to each family. and will meet with the parent/guardian and the adolescent in the home or community once a week to address concerns.
The Cognizant Me Mentorship Program strives to make our students and communities accountable through conflict resolution and mentorship! We promote emotional regulation, responsibility, academic achievement, career development, and personal growth through one-to-one or group mentoring. Our mentors involve our students and the community in enrichment activities, academic tutoring, school advocacy, cultural activities, and life skills development.
As violence increases in schools and throughout our city, it is time to give our children the tools they need to resolve disputes peacefully. Physical harm, gun violence, and deaths impact our communities daily. This has led to our youth struggling to cope and feel safe. Cognizant Me Conflict Resolution Programming provides professionals trained in restorative response to work with schools, students, and the community to teach accountability.
The goal is to instill lifelong skills that help students overcome conflicts and build positive relationships. These services have been proven to increase school performance, lower incidents of harm, and reduce suspensions and expulsion rates.
Careful Crossing Program is a community-based initiative that guides students as they commute to and from school. Fostering a positive school experience is our priority!
Careful Crossing Facilitators (CCF’s) are trained, trusted adults and local businesses committed to keeping the youth safe. Our Careful Crossing Facilitators and trusted business partners keep a watchful eye on students from the time they leave home in the morning until they return in the evening.
Through a unique combination of in-school and out-of-school supports, Careful Crossing ensures students feel safe physically and emotionally. In addition to assisting students as they commute, the program offers community engagement, conflict resolution and peer mediation. We are able to accomplish this mission by building strong relationships and maintaining clear communication with businesses, schools, families and community members.
